Vet AI Scribe for Relief Vets

Your scribe belongs to you, not the clinic. Record on your phone or in the browser, get the note in the format that clinic expects, and paste it into whichever PIMS they run.

What PawfectNotes Does for Relief Vets

  • One account, every clinic

Sign in on your phone or any browser on the shift. Your templates and preferences come with you, whichever clinic you're covering this week.

Subjective

Chief Complaint

Head shaking and scratching at the right ear for four days. Owner noticed a smell from the ear yesterday.

History

  • 5-year-old neutered male golden retriever.
  • Swims in the lake most weekends; last swim six days ago.
  • Per the clinic record, one previous episode of right otitis externa in March 2026, treated by Dr. Patel with a topical otic; resolved at recheck.
  • Licks his front paws in summer; no other skin history.
  • Eating and drinking normally. No vomiting or diarrhea.
  • Current medications: monthly heartworm and flea/tick prevention only.

Objective

Vitals

  • Weight: 34.6 kg (34.1 kg in March)
  • Temperature: 101.9ยฐF
  • Heart rate: 96 bpm
  • Respiratory rate: Panting
  • Body condition score: 5/9

Physical Exam

  • Right ear: Pinna erythematous; vertical canal swollen and erythematous with moderate brown-yellow exudate and a strong odor. Painful on manipulation of the pinna. Tympanic membrane not visible before cleaning.
  • Left ear: Mild ceruminous debris; canal and tympanic membrane normal.
  • Skin: Mild interdigital erythema on both forepaws; no other lesions.
  • Lymph nodes: Normal size.
  • Remainder of the exam unremarkable.

Diagnostics

  • Ear cytology, right: Many cocci (more than 25 per oil-immersion field), moderate Malassezia (8โ€“10 per field), rare neutrophils, no rods.
  • Ear cytology, left: Occasional Malassezia; no bacteria.
  • After in-clinic flush: Right tympanic membrane visualized and intact.

Assessment

  1. Right otitis externa, cocci and Malassezia โ€” second episode in seven months.
  2. Possible underlying allergic skin disease โ€” seasonal paw licking and recurrence point that way; swimming is likely a contributing factor.

Plan

  • Right ear flushed in clinic with warm saline after cytology.
  • Florfenicol/terbinafine/betamethasone acetate otic gel: one tube in the right ear today; second tube in 7 days.
  • Carprofen: 75 mg PO q12h with food for 3 days, for ear pain.
  • Do not clean the ear at home between doses.
  • No swimming until the recheck shows the ear has cleared.

Handover for Dr. Patel

  • Second tube due Saturday, October 24; repeat cytology at that visit.
  • If cocci persist at the recheck, consider culture and susceptibility.
  • Second episode this year: the owner is open to talking about an allergy workup.

Client Communication:
Explained that this is the same ear as in March and that the medication stays in the ear for the week, so the ear should not be cleaned at home. Discussed that a second infection in a year suggests something underneath, often allergy, and that Dr. Patel can go through the options at the recheck. Owner agreed to keep him out of the lake for now.

Relief Vets FAQ

Does the clinic need its own PawfectNotes account?

No. You can use your own account on any shift and paste the note into the clinic's PIMS. Check first that the clinic is happy for you to use an AI scribe and to record appointments; our free consent form covers the client side.

Can I keep a different note format for each clinic?

Yes. Make a custom template for each clinic, name it after the clinic, and choose it before you record. Library templates are a quick starting point.

Is the free plan enough for relief work?

For a few shifts a month it often is: FreeNotes includes 50 sessions a month, never expires and needs no credit card. If you go over, StarterNotes gives you 100 sessions a month for $29/vet/month billed annually ($39 billed monthly).

What if the clinic runs a desktop PIMS?

There is no desktop app to install. Open PawfectNotes in a browser or on your phone next to the PIMS and copy the note across. The integration pages show how for each system.
